When you submit this form, the recognition will be received by a Human Resources Office employee who will process your nomination.

Once the nomination has been processed, an electronic recognition certificate will be emailed to the nominee. The nominee's supervisor will also receive email notification and the person submitting the nomination will receive an acknowledgement.
