Change of Student Information

If you moved, changed your name or your phone number, remember to update this information with the College. All changes to a student's personal information must be in writing. You may download a PDF version of the change of student information form (30 KB, 1 page).

*Note: You must download and install Adobe® Acrobat® Reader™ in order to view and print the change of student information form.

After completing and signing the form, you may drop it off, mail or fax it to:
